Maybe the most extensive breast cancer I’ve seen and in a male patient
66M patient was referred for a breast and lymph node biopsy. Patient was actually seen at a different clinic back in 2022 and was advised to have a biopsy then which he did not ever have done. He came back for imaging last month now that he has an open wound on his chest. Ultrasound showed a mass that’s 7.2 x 5.8 x 7cm. The mammo shows the full extent of the mass.

ER Doc originally ordered a hand and wrist with it as well, but we told them we were only going to leave the collimation open and include what we could in these 2 images. The kid was like 9 and wasn't crying, surprisingly
10yo male with progressive macroglossia.
Patient was first seen at 2yo when his tongue started getting big. Parents cant recall what was advised to be done at the time and didnt seek further evaluation for the kid. Over the next 8 years no consult was done despite slow enlargement because patient could still eat and breath properly. Came to the OPD when the tongue was so large that eating was already difficult. CT scan showed a tongue large enough to fill the entire oral cavity. Mandibular teeth were splayed outward due to the tongue size. Nasopharynx and oropharynx still open. Contrast showed what looked like venous malformations of the right lateral neck with a big nidus in the superficial lobe of the right parotid gland. Patient is currentlt scheduled for venography.

The other day there was a patient on the scanning table, nothing crazy, just a chest without. Anyway, an ICU nurse came to the CT room with a critical patient, without calling ahead to let us know mind you and was getting irritated that we weren't quite ready. He kept opening the door during the scan and walking in, maybe 4 or 5 times to check in. By the time the scan finished, patient is off the table and wheeled back to the ED, I turned to the nurse and said "You know you got hit with a half lethal dose of radiation right?" He went completely white and started asking me if I was kidding to which I told him I was. There was a Student Nurse with him and she experienced the whole thing. End of shift rolls around and as I'm leaving, the Student Nurse catches me in the hallway and pulls me aside to tell me that guy was freaking out for about three hours after our interaction. I guess she thought it was hilarious.

AI is creeping into every corner of radiology and our reads are silently fueling someone else’s algorithm and profits at the peril of our work future. We have a window of opportunity to maintain control. With the market in our favor, we need a concerted effort to: Lock It Down in Contracts Add clauses that ban the use of your reports/images for AI training without explicit consent. Own your interpretations—spell it out in your services agreement. 2. Tag Your Work Use PACS or DICOM metadata to flag studies: “Not for AI training.” It’s not foolproof, but it sends a signal. 3. Ask the Right Questions Who are your hospital or telerad vendors partnering with? Are they feeding your work into the next ChatGPT of radiology? 4. Push for Transparency Advocate for opt-out policies and ethical use audits. Join forces with your group to demand visibility. Your intellectual property is training AI. We should know about it, and at the least get paid for it.
